ELIGIBILITY FOR THE AWARD
EXAMPLES
A) Achievements in the furtherance of the rights of disabled people.
B) A significant personal achievement in the life of an individual.
CRITERIA
The award will be made based on the evidence of one or many of the following:
- Commitment and contribution to furthering the equality, social inclusion and rights of people with disabilities.
- Contribution to a wider community understanding of disability and disability issues.
- Significant and diligent work on a particularly important disability issue or campaign.
- Work done over the above professional or personal commitments (e.g. unpaid or dedicated work).
The judges will look at the impact of the nominated individuals achievement/s.

Nominations must be submitted using the official nomination form.

Judges will not consider any additional information and will rely solely on the statement contained in the nomination form.

Any individual or organisation can make nominations except the panel of judges.
Self nomination is not allowed.
Nominations should clearly state the major achievements made by the individuals.
